    A profiling analysis of contributions of cigarette smoking, dietary calcium intakes, and physical activity to fragility fracture in the elderly

    Fragility fracture and bone mineral density (BMD) are influenced by common and modifiable lifestyle factors. In this study, we sought to define the contribution of lifestyle factors to fracture risk by using a profiling approach. The study involved 1683 women and 1010 men (50+ years old, followed up for up to 20 years). The incidence of new fractures was ascertained by X-ray reports. A “lifestyle risk score” (LRS) was derived as the weighted sum of effects of dietary calcium intake, physical activity index, and cigarette smoking. Each individual had a unique LRS, with higher scores being associated with a healthier lifestyle. Baseline values of lifestyle factors were assessed. In either men or women, individuals with a fracture had a significantly lower age-adjusted LRS than those without a fracture. In men, each unit lower in LRS was associated with a 66% increase in the risk of total fracture (non-adjusted hazard ratio [HR] 1.66; 95% CI, 1.26 to 2.20) and still significant after adjusting for age, weight or BMD. However, in women, the association was uncertain (HR 1.30; 95% CI, 1.11 to 1.53). These data suggest that unhealthy lifestyle habits are associated with an increased risk of fracture in men, but not in women, and that the association is mediated by BMD

    On the predictions for diffusion-driven evaporation of sessile droplets with interface cooling

    The diffusion-driven evaporation of sessile droplets from planar surfaces is influenced by cooling at the air-liquid interface. Here, corrections to the available models for predicting the evaporation process are presented. The mass conservation for diffusion-driven evaporation is resolved by considering the effect of interface cooling on the change in density of saturated vapour along the liquid-vapour interface of sessile droplets. Corrections to the predictions for the spatial distribution of vapour density around a sessile droplet and the evaporative flux of vapour at the interface are obtained. The classical models are recovered from the new predictions if interface cooling is negligible. Comparison between the new and classical predictions for the local surface evaporative flux is obtained using the literature data. Our analysis shows a significant effect of interface cooling which should be considered in predicting diffusion-driven evaporation of sessile droplets on planar surfaces

    Seismic Interpretation of the Nam Con Son Basin and Its Implication for the Tectonic Evolution

    DOI:10.17014/ijog.3.2.127-137The Nam Con Son Basin covering an area of circa 110,000 km2 is characterized by complex tectonic settings of the basin which has not fully been understood. Multiple faults allowed favourable migration passageways for hydrocarbons to go in and out of traps. Despite a large amount of newly acquired seismic and well data there is no significant update on the tectonic evolution and history of the basin development. In this study, the vast amount of seismic and well data were integrated and reinterpreted to define the key structural events in the Nam Con Son Basin. The results show that the basin has undergone two extentional phases. The first N - S extensional phase terminated at around 30 M.a. forming E - W trending grabens which are complicated by multiple half grabens filled by Lower Oligocene sediments. These grabens were reactivated during the second NW - SE extension (Middle Miocene), that resulted from the progressive propagation of NE-SW listric fault from the middle part of the grabens to the margins, and the large scale building up of roll-over structure. Further to the SW, the faults of the second extentional phase turn to NNE-SSW and ultimately N - S in the SW edge of the basin. Most of the fault systems were inactive by Upper Miocene except for the N - S fault system which is still active until recent time

    Suicide attempt in a rural area of Vietnam: Incidence, methods used and access to mental health care

    OBJECTIVES: The study aims to determine the incidence of suicide attempt, describe the methods used, and assess use of health care services including mental health care after suicide attempt in a rural area of Vietnam. METHODS: All suicide attempters (104) during 2003-2007 were listed, diagnosed and re-evaluated by trained physicians according to the research criteria of the WHO Multicentre Study of Attempted Suicide. All attempters were interviewed by trained medical staff to investigate methods used, socio-demographic characteristics and use of health services. RESULTS: The yearly incidence was 10.2 per 100000 person-years, 10.6 per 100000 in males and 9.8 per 100000 in females. 99% of cases committed suicide attempt by poisoning, 62.6% by pesticides and 36.3% by pharmaceutical drugs. 34.3% reported having been in contact with somatic care and 13.2% had received mental health care. Among those who reported some treatment received, 47.5% had been in contact with official health care services, 8.1% had pharmacy keepers' consultation or were treated by traditional healers and 4% reported self treatment. CONCLUSION: The incidence of suicide attempt was lower in this population compared to other settings. While the majority of attempters use pesticides, many had used psychotropic drugs. Contact with mental health services following the attempt was very limited in this setting. Suicide prevention for this high risk group should focus on reducing access to pesticides and psychotropic drugs. Mental health services should be made more accessible in rural areas

    Effects of ASE noise and dispersion chromatic on performance of DWDM networks using distributed Raman amplifiers

    We investigate effects of amplified spontaneous emission noise (ASE), noise figure (NF) and dispersion chromatic on the performance of DWDM networks using distributed optical fiber Raman amplifiers (DRAs) in two different pump configurations, i.e., forward and backward pumping. We found that the pumping configurations, ASE noise, and dispersion play an important role in network performance improving since it reduces noise figure and bit error rate (BER) of the system. Simulation results show that the lowest bit error rate and noise figure when using forward pumping configuration.     Relative Contributions of Lean and Fat Mass to Bone Mineral Density: Insight From Prader-Willi Syndrome

    Context: Low bone mineral density (BMD) is the most important risk factor for fragility fracture. Body weight is a simple screening predictor of difference in BMD between individuals. However, it is not clear which component of body weight, lean (LM), or fat mass (FM), is associated with BMD. People with the genetic disorder of Prader-Willi syndrome (PWS) uniquely have a reduced LM despite increased FM.Objective: We sought to define the individual impact of LM and FM on BMD by investigating subjects with and without PWS.Design, Setting and Participants: This cross-sectional study was conducted at the Clinical Research Facility of the Garvan Institute of Medical Research, with PWS and control participants recruited from a specialized PWS clinic and from the general public by advertisement, respectively. The study involved 11 adults with PWS, who were age- and sex-matched with 12 obese individuals (Obese group) and 10 lean individuals (Lean group).Main Outcome Measures: Whole body BMD was measured by dual-energy X-ray absorptiometry. Total body FM and LM were derived from the whole body scan. Differences in BMD between groups were assessed by the analysis of covariance model, taking into account the effects of LM and FM.Results: The PWS group had significantly shorter height than the lean and obese groups. As expected, there was no significant difference in FM between the Obese and PWS group, and no significant difference in LM between the Lean and PWS group. However, obese individuals had greater LM than lean individuals. BMD in lean individuals was significantly lower than in PWS individuals (1.13 g/cm2 vs. 1.21 g/cm2, p < 0.05) and obese individuals (1.13 g/cm2 vs. 1.25 g/cm2, p < 0.05). After adjusting for both LM and FM, there was no significant difference in BMD between groups, and the only significant predictor of BMD was LM.Conclusions: These data from the human genetic model Prader-Willi syndrome suggest that LM is a stronger determinant of BMD than fat mass

    Infant and Young Child Feeding Practices Differ by Ethnicity of Vietnamese Mothers

    Background: Limited studies have examined ethnic variation in breastfeeding and complementary feeding practices in developing countries. This study investigated ethnic variation in feeding practices in mothers with children 0–23 months old in Vietnam. Methods: We used data on 1875 women who came from the ethnic majority, Kinh (n = 989, randomly sampled from 9875 surveyed Kinh mothers, 10 % from each province) and three ethnic minorities: E De-Mnong (n = 309), Thai-Muong (n = 229) and Tay-Nung (n = 348). Ethnic minorities were compared with the Kinh group using logistic regression model. Results: Prevalence of breastfeeding initiation within an hour of birth was 69 % in Thai-Muong, but ~50 % in other ethnicities. In logistic regression, the prevalence of breastfeeding within one hour was lower in Tay-Nung (OR: 0.54; 95 % CI: 0.38, 0.77) than the majority Kinh. Prevalence of exclusive breastfeeding under 6 months was 18, 10, 17, and 33 % in Kinh, Thai-Muong, Tay-Nung, and E De-Mnong, respectively; compared to the majority Kinh, the prevalence was lower in Thai-Muong (OR: 0.42; 95 % CI: 0.25, 0.71) and higher in E De-Mnong (OR: 1.99; 95 % CI: 1.04, 3.82). Overall prevalence of bottle feeding in Thai-Muong and E De-Mnong (~20 %) was lower than in Kinh (~33 %): Thai-Muong (OR: 0.50; 95 % CI: 0.37, 0.68) and E De-Mnong (OR: 0.69; 95 % CI: 0.50, 0.95). Compared with Kinh (75 %), fewer ethnic minority children received minimum acceptable diets (33 % in Thai-Muong, 46 % in E De-Mnong, and 52 % in Tay-Nung; P \u3c 0.05). Prevalence of minimum acceptable diet (met both dietary frequency and diversity) was lower in Thai-Muong (OR: 0.23; 95 % CI: 0.11, 0.46), Tay-Nung (OR: 0.52; 95 % CI: 0.39, 0.69), and E De-Mnong (OR: 0.55; 95 % CI: 0.33, 0.89) than the majority Kinh. Conclusions: Breastfeeding practices were suboptimal and differed by ethnicity, which suggests need for tailored interventions at multiple levels to address ethnic-specific challenges and norms. Complementary feeding practices were less optimal among ethnic minorities compared to Kinh, which suggests need for broad intervention including improved food availability, accessibility, and security

    Context modeling of agile software and a contextbased approach to support virtual enterprises

    In the practice of software development contexts have been only implicitly modeled and transformed in fixed part of software. The information about context is dispersed in objects across the application. Each context change leads to modification or new development of the software. The context modeling helps developer to separate context objects from contextdependent objects. It allows better reuse of analysis, design and implementation models, if the context of certain objects is changed. The context modeling is interesting and necessary, when the software should be agile - i.e. when the environment and the condition of the software could be changed permanently, e.g. in case of platform for virtual enterprises. The paper introduces a novel approach to support processes within generic platforms for virtual enterprises: the context-based approach. The main advantage of the approach lies in its generic capacity, which allows the users to define processes flexibly to support their own enterprises. In this paper, we discuss further the phenomenon of extra-context logic, its modeling and its application case. The information of extra-context logic provides not only the better understanding of application domain, but also can be used by a wizard to support the interaction of users working with multiple systems

    Destination-aware Adaptive Traffic Flow Rule Aggregation in Software-Defined Networks

    In this paper, we propose a destination-aware adaptive traffic flow rule aggregation (DATA) mechanism for facilitating traffic flow monitoring in SDN-based networks. This method adapts the number of flow table entries in SDN switches according to the level of detail of traffic flow information that other mechanisms (e.g. for traffic engineering, traffic monitoring, intrusion detection) require. It also prevents performance degradation of the SDN switches by keeping the number of flow table entries well below a critical level. This level is not preset as a hard threshold but learned during operation by using a machine-learning based algorithm. The DATA method is implemented within a RESTful application (DATA App) which monitors and analyzes the ongoing network traffic and provides instructions to the SDN controller to adapt the traffic flow matching strategies accordingly. A thorough performance evaluation of DATA is conducted in an SDN emulation environment. The results show that---compared to the default behavior of common SDN controllers---the proposed DATA approach yields significant SDN switch performance improvements while still providing detailed traffic flow information on demand.Comment: This paper was presented at NetSys conference 2019. arXiv admin note: text overlap with arXiv:1909.0154
